Support for Care Partners Navigating Care

Caring for a loved one with Alzheimer’s, Parkinson’s, dementia, or another neurological condition can be overwhelming.

Family Care Connect provides guidance, education, and support to help caregivers navigate both immediate decisions and long-term planning—while also prioritizing their own well-being.

Care Consultations

Licensed social workers provide guidance on care planning, available resources, and strategies for managing the challenges of caregiving.

Memory CAFÉ

For care partners and loved ones living with dementia. Enjoy a welcoming afternoon with lunch and time together.

Educational Workshop Series

A professionally facilitated seminar series designed for spouses and adult children caring for loved ones with dementia.

Topics include:

  • communication strategies
  • behavior management
  • coping techniques
  • caregiver self-care

Professionally facilitated support groups (10 Week Semesters)

Adult Children Care Partner 

Support for those in their 20s and 30s with a parent diagnosed with Dementia, Parkinson’s disease, or another neurodegenerative illness.

Spousal Care Partner 

Support for those with a spouse diagnosed with dementia, Parkinson’s, or another neurodegenerative illness.

An annual event featuring expert speakers, panel discussions, and resources for caregivers and professionals.

National Family Caregiver Support Program (NFCSP)

The National Family Caregiver Support Program provides support, education, and resources to family and informal caregivers caring for older adults

Services include:

  • Home care stipends
  • Adult day programs
  • Care management support
  • Support groups
  • Educational opportunities
  • Assistive devices
  • Meals
  • Minor home adaptations

Communities served by the National Family Caregiver Support Program include Albertson, Bellerose, East Hills, Floral Park, Great Neck, Manhasset, Mineola, Port Washington, Roslyn, Williston Park, and surrounding communities.
