Information & Assistance

INFORMATION AND ASSISTANCE

CONNECTION TO COMMUNITY RESOURCES
The Social Work Team is here to help the community navigate, learn, and thrive in expected and unexpected ways. We will conduct a brief intake to gain an understanding of the life’s challenges you are facing and connect you to local resources to support your needs.  These services include but are not limited to financial & legal assistance, government benefits, social programs, and support groups.

MENTAL HEALTH + WELLNESS CLINICAL SUPPORT
Our licensed social workers, where appropriate, will refer to short-term counseling for community members who are uninsured or underinsured. Offered by our on-site partner, Central Nassau Guidance and Counseling

ISOLATION TO CONNECTION
For community members who do not currently participate in Sid Jacobson JCC’s senior programming, we have partnered with UJA-Federation of New York to provide short-term social work services and personalized connection with long-term resources to reduce isolation.

FOOD INSECURITY ASSISTANCE
We assist with buying groceries in a dignified setting.
